Destiny Color Guard Participant Waiver and Release of Liability

Thank you for registering your student for Destiny Color Guard. Please read and acknowledge the following important information to ensure a safe and positive experience for all participants.
1. Voluntary Participation
I understand that participation in Destiny Color Guard is voluntary and includes physical activity such as basic movement, marching, and the use of flags, ribbons, and other equipment.
2. Assumption of Risk
I recognize that physical activity carries inherent risks, including but not limited to minor injuries, slips, falls, or accidental contact with equipment. I voluntarily assume all risks associated with my child’s participation in this program.
3. Release of Liability
In consideration of my child being allowed to participate, I hereby release, waive, and hold harmless FUSE, Inc., its staff, board members, volunteers, partnering facilities, and representatives from any and all liability, claims, or demands related to injuries, damages, or losses arising from participation in Destiny Color Guard.
4. Medical Authorization
In the event of illness or injury, I authorize FUSE, Inc. staff or volunteers to provide basic first aid and seek emergency medical treatment if necessary. I understand that I am financially responsible for any medical care provided.
5. Behavior and Safety Expectations
I understand that my child is expected to follow staff directions and safety rules to ensure the well-being of all participants. Repeated unsafe behaviors may result in removal from the activity for the safety of the group.
6. Photography and Media Release
I give permission for FUSE, Inc. to use photos or videos of my child taken during practices or performances for promotional or informational purposes. If I do not consent, I will notify staff in writing.
7. Transportation
I understand that transportation to and from practices and performances is the responsibility of the parent/guardian unless otherwise noted.
